About Health Partners Plans
Health Partners Plans is an award-winning, not-for-profit health insurance organization serving more than 165,000 members in Philadelphia, Chester, Delaware, Bucks and Montgomery counties.
We offer Medicaid and Children’s Health Insurance Program (CHIP) plans that include special benefits to improve the health and wellness of our members. We’re also committed to boosting the health of our community through outreach, education and events.
Health Partners Plans network includes:

Our Health insurance plans
We offer comprehensive health care through two high-quality plans:
Health Partners: Our HealthChoices Medicaid plan consistently ranks number one in the region for member satisfaction in an annual consumer survey and is recognized as a national model for Medicaid managed care.
KidzPartners: Our CHIP plan, that provides coverage for children up to age 19 who are uninsured and not eligible for Medicaid. No family makes too much money for CHIP. For most families, CHIP is free -- for others, low cost.
Medicare: Health Partners Medicare offers four Medicare Advantage plans in Philadelphia, all of which provide more benefits than Original Medicare with no or low monthly plan premiums.
